Questions

01Is there a free esthetician consent form?

Yes. The printable form on this page covers adhesive, latex and fragrance allergies, recent eye surgery, contact lenses, pregnancy, retinoid use and active skin conditions, with a consent statement and a signature line. It is a starting point, not legal advice.

02Does the same form work for eyelash extensions and microdermabrasion?

Yes. The esthetician preset asks the contraindication questions both services need. If you want a separate form per service, that is a custom job we quote.

03Can clients sign the form when they book online?

Yes. The booking software shows the form before the calendar, records the consent with a typed signature and a timestamp, and can take a deposit.
